Discover Nursing Australia is a nurse-led disability support and community nursing provider committed to supporting people living with disability to achieve greater independence, dignity, safety and choice. We provide specialised nursing and clinical services to participants receiving support through the National Disability Insurance Scheme (NDIS), including participants living independently, with family or in supported accommodation.
We currently have an opportunity for an experienced Registered Nurse, Clinical Nurse Consultant, Continence Nurse or Clinical Nurse to provide clinical services to a participant located at: Moama NSW 2731.
This opportunity is offered on a contractual (sole trader/ABN) basis, providing you with greater flexibility and control over when you work and the clinical services you undertake. The role would particularly suit an experienced Registered Nurse who enjoys community-based nursing, clinical assessment, care planning and working autonomously with people who have disability and complex support needs.
The Role:
Depending on your qualifications, clinical experience and the individual requirements of the participant, responsibilities may include:
- Conducting comprehensive and holistic nursing assessments.
- Continence assessments and management planning.
- Complex health and disability assessments.
- Clinical risk assessments.
- Developing individualised nursing and clinical care plans.
- Reviewing existing clinical support plans.
- Assessing personal care and high-intensity support requirements.
- Medication-related assessment and clinical review.
- Skin integrity and pressure care assessment.
- Bowel and bladder management.
- Complex continence support.
- Enteral feeding and PEG-related assessment where applicable.
- Dysphagia and mealtime-related clinical considerations within nursing scope.
- Seizure and epilepsy-related nursing assessment and care planning.
- Respiratory and complex health support planning.
- Identifying clinical risks and appropriate escalation pathways.
- Preparing professional nursing reports and recommendations.
- Providing education, guidance and clinical support to Disability Support Workers where appropriate.
- Liaising with participants, families, Support Coordinators, allied health professionals, GPs and other members of the participant's healthcare team.
Clinical services will be undertaken within the Registered Nurse's professional scope of practice, experience and competency.
About You:
We are looking for a Registered Nurse who is:
- Experienced and clinically confident.
- Comfortable working autonomously in a community setting.
- Experienced in comprehensive nursing assessment and care planning.
- Able to identify and appropriately manage clinical risk.
- Professional and confident communicating with participants and families.
- Able to prepare clear, comprehensive clinical documentation.
- Familiar with multidisciplinary care and professional collaboration.
- Reliable, organised and responsive.
- Committed to person-centred and evidence-informed practice.
Experience in disability, continence, community nursing, rehabilitation, complex care, high-intensity supports or NDIS clinical assessments will be highly regarded.

Eligibility Criteria:
Applicants must have, or be willing to obtain where applicable:
- Current AHPRA registration as a Registered Nurse.
- Bachelor of Nursing or recognised equivalent qualification.
- Evidence of current Australian working rights.
- Australian Business Number (ABN) or willingness to obtain one.
- Current NDIS Worker Screening Check.
- Current National Police Check.
- Current Working with Children Check (where required).
- Current First Aid Certificate.
- Current CPR certification.
- Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
- Current driver's licence and reliable transport.
- Strong written clinical documentation and communication skills.
- Ability to work autonomously and within professional scope of practice.
